23 Strange Signs Your Kidneys Might Be in Trouble

Your kidneys help filter waste from your blood, balance fluids, regulate blood pressure, and support healthy blood production. Kidney problems can develop slowly, and early symptoms may be easy to miss.

Having one of these signs does not automatically mean you have kidney disease, but persistent or unusual changes should be discussed with a healthcare professional.

1. Changes in urination

Possible warning signs include:

  • Urinating much more or less than usual
  • Needing to urinate frequently at night
  • Difficulty starting urination
  • Changes in urine color

2. Foamy urine

Persistent foam or bubbles in urine may sometimes indicate excess protein in the urine, which can be a sign of kidney damage.

3. Blood in the urine

Blood in urine should always be evaluated, as it can have many causes, including kidney problems.

4. Swelling in the feet, ankles, or legs

When kidneys struggle to remove extra fluid, swelling (edema) may occur.

5. Puffy eyes

Fluid retention can sometimes cause swelling around the eyes, especially in the morning.

6. Unusual tiredness

Kidney problems can contribute to anemia and waste buildup, which may cause:

  • Fatigue
  • Weakness
  • Reduced energy

7. Trouble concentrating

Some people with more advanced kidney disease experience difficulty with focus or mental clarity.

8. Poor appetite

Changes in appetite or a reduced desire to eat can occur.

9. Nausea or vomiting

Waste buildup in the bloodstream can affect digestion in advanced kidney disease.

10. Itchy skin

Kidney-related mineral and waste imbalances may contribute to persistent itching.

11. Dry skin

Changes in fluid balance can cause skin dryness.

12. Muscle cramps

Electrolyte changes may contribute to cramps or muscle discomfort.

13. Shortness of breath

Fluid buildup or anemia may affect breathing.

14. High blood pressure

Kidneys and blood pressure are closely connected. Kidney disease can contribute to difficult-to-control hypertension.

15. Pain in the side or back

Kidney stones, infections, or other kidney conditions can cause pain in the flank area.

16. Fever with urinary symptoms

Fever, pain during urination, or back pain may suggest a kidney infection.

17. Bad taste in the mouth

Some people with advanced kidney problems notice a metallic taste or changes in taste.

18. Bad breath

Waste buildup can sometimes cause unusual breath odor.

19. Sleep problems

Itching, discomfort, restless legs, or frequent nighttime urination may disturb sleep.

20. Numbness or tingling

Nerve problems can occur in some people with advanced kidney disease.

21. Unexpected weight changes

  • Weight gain from fluid retention
  • Weight loss from reduced appetite or illness

22. Frequent infections or slow healing

Certain health conditions linked with kidney disease may affect immunity and healing.

23. Abnormal kidney test results

Kidney problems are often detected through routine tests before major symptoms appear:

  • Blood tests (such as creatinine and eGFR)
  • Urine tests (checking protein or blood)

Who is at higher risk of kidney disease?

Risk is higher with:

  • Diabetes
  • High blood pressure
  • Older age
  • Family history of kidney disease
  • Heart disease
  • Long-term use of certain medications, including some pain relievers

Seek medical care urgently for:

  • Very little or no urine
  • Severe swelling
  • Chest pain or severe breathing problems
  • Confusion
  • Blood in urine with severe pain or fever

Regular checkups, controlling blood pressure and blood sugar, staying active, and avoiding unnecessary medication risks are key steps for protecting kidney health.
